Scales Mound CUSD #211 is a Pre K-12 grade public school district 70 square  miles in area, all of which is located in northern Jo Daviess County. The District  is positioned approximately 25 miles east of Dubuque, Iowa and 80 miles  southwest of Madison, Wisconsin. The areas making up the School District  include the Village of Scales Mound, Apple Canyon Lake and the  Galena Territory. Our enrollment is less than 300 students with an elementary  class size average of less than 15 students per section and a pupil to teacher  ratio of approximately 13 to 1. All Scales Mound Student are housed under a  single school building. The district owns nor lease any other facilities. The  District offers a rich curriculum and a number of co-curricular opportunities.  Scales Mound students have traditionally scored well on the Illinois Standards  Achievement Test and the Iowa Test of Basic Skills.

US NewsScales Mound High School has once again been recognized by the U.S. News and World Report as one of America’s Best High Schools.  Analysis for this award is based on standardized test performance in reading and mathematics, proficiency rates for disadvantaged students, high school graduation rate, and college-readiness performance.

Health Examination Requirements for Grades PK, K, 6 & 9:

For students entering PreK, kindergarten, 6th grade and 9th grade, or any new students entering the district for the first time, the State of Illinois requires that a Certificate of Child Health Examination be completed.  This examination should include up-to-date immunization records.  Sports physicals do not meet this requirement.   However, on the Certificate of Child Health Examination there is a section entitled Interscholastic Sports.  Your child’s physician must answer yes, no, or limited.  If properly completed, this section of the Certificate of Child Health Examination satisfies the requirement for a sport physical.

Please see the attached Immunization Requirements for students entering an Illinois School for Fall-2017. REMINDER - Students entering 6th, 7th, 8th and 12th are required to receive the Meningococcal Vaccine.

NEW REQUIREMENT FOR 2017-2018 - Health examination must include an age-appropriate developmental screening and an age-appropriate social and emotional screening. The Illinois Department of Public Health is responsible for drafting rules and changes to the Child Health Examination form related to this Public Act.  Once this form is available we will forward it to parents.

Dental Requirement for Grades K, 2 & 6

 All Illinois children in Kindergarten, 2nd grade, and 6th grade are required to have an examination performed by a licensed dentist.  School dental examinations must have been completed within the 18 months prior to the May 15th deadline.  If the student in second or sixth grade fails to present proof by May 15th, the school may hold the child’s report card until one of the following occurs:  the child presents proof of a completed dental examination or the child presents proof that a dental examination will take place within 60 days after May 15th.

Eye Examination Requirements for all Illinois children in kindergarten or upon first entry into an Illinois school beyond kindergarten (i.e. 1-12):

All children in kindergarten or entering any grade above kindergarten for the first time in an Illinois school must present proof of having been examined by a licensed optometrist or ophthalmologist.  He/she shall complete and sign the Eye Examination Report form.  School eye examinations must have been completed within the 12 months prior to the October 15 deadline or the child presents proof of an appointment for an eye examination scheduled within 60 days after the October deadline.
